Warning: Contains SPOILERS for The Last of Us season 2, episode 5!
The Last of Us season 1 was remarkably accurate to the source material, with its only major changes being additional stories added in to flesh out side characters. However, the same can't be said for The Last of Us season 2, which has altered The Last of Us Part II in some big ways. The Last of Us season 2, episode 5 is continuing this trend, with the Lakehill Seattle Hospital story containing these eight changes.
The Last of Us season 2, episode 4 ended with Ellie learning that Dina is pregnant, Dina learning that Ellie is immune, and the duo setting their sights on Lakehill Seattle Hospital. In episode 5, they make their way there, hoping to find more information about Abby and the WLF. Unfortunately, they are attacked by a gang of Stalkers. Jesse appears and saves them, revealing that he and Tommy came to Seattle in order to look for Ellie and Dina. After a chase with the WLF and the Seraphites, Ellie ends up going into Lakehill alone, where she encounters Nora.
8 Episode 5 Opens With A New WLF Scene
Explaining The Origin Of Spores
In The Last of Us video games, the player is always stuck in the perspective of the playable characters. Thus, if a character has something going on when Joel, Ellie, or Abby aren't around, the player doesn't get to see it. HBO's The Last of Us doesn't have this restriction, meaning that it can have scenes featuring almost anyone. The Last of Us season 2, episode 5 proves this, with its opening scene focusing on two yet unseen WLF .
The scene is an interrogation, with one WLF member asking another what happened to her squadron. The latter explains that she sent a group of soldiers into the basement of Lakehill Seattle Hospital, where they made a horrifying discovery. They all became infected through an airborne version of the Cordyceps virus, setting up the return of spores. This scene isn't in the original game due to none of The Last of Us' main characters being around, as well as spores already being a pre-established part of the world by this point.
7 Spores Have Only Now Been Added To The HBO Show
After Being Removed From Season 1
Spores are one of the biggest gameplay elements in The Last of Us video games, yet they are just now being added to the HBO series. In the games, spores are airborne particles of the Cordyceps virus, with various patches of fungus spreading them. Because of spores, characters in The Last of Us are often seen wearing gas masks, preventing them from becoming infected in this way.
For a variety of reasons, spores were removed from The Last of Us season 1, meaning that being bitten was the only way to become an Infected. However, The Last of Us season 2, episode 5 sees some WLF discover spores in the basement of the Lakehill Seattle Hospital. They explain that these basement floors were ground zero for Seattle's Cordyceps infection, setting up the possibility that spores are a highly-evolved form of the Cordyceps virus.
6 Dina's Sister Has A Different Backstory
Talia Is Killed Much Earlier
Dina decides to tell Ellie a bit about her backstory in episode 5, telling her about the first person she killed. Dina explains that she grew up in a cabin north of Santa Fe, living with her mom and sister. One day, while Dina was playing outside, a raider entered her home. Dina heard her mom and sister screaming, causing her to run home. Dina didn't arrive until after the raider killed them, with her then shooting the raider.
This is a big difference from Dina's backstory in The Last of Us Part II. In the game, Dina's mother is still killed by a non-Infected human. However, her sister isn't. Instead, her sister is a character named Talia, with her raising Dina after they are orphaned. Talia died due to unknown reasons years later. So, unless Dina has two sisters, it seems that the HBO series killed of Talia at the same time as their mother.
5 Ellie Doesn't Go To Lakehill Alone
Dina Doesn't Stay At The Theater
One of the biggest changes in The Last of Us season 2 so far has been how actively Dina is taking part in the story, something that continues in episode 5. In the game, Ellie leaves Dina behind at the theater after discovering that she is pregnant. Ellie decides to head to Lakehill on her own, with her only coming back after killing Nora.

What Happens To Dina & [SPOILER] In The Last Of Us
The Last of Us season 2 introduces an important new variable in the characters' futures that may leave viewers wondering where Ellie and Dina end up.
Things play out differently in the HBO series. Dina comes along with Ellie, with the duo planning on going to Lakehill together. Unfortunately, the encounters with the Stalkers, the WLF, and the Seraphites change their plans. Ellie winds up at Lakehill alone, with her sending Dina and Jesse back to the theater. However, the fact that Dina left the theater at all during the Lakehill arc is a big change.
4 Jesse & Tommy Traveled To Seattle Together
Since Tommy Wasn't The First One To Leave Jackson
One of the biggest changes in HBO's The Last of Us season 2 so far has had to do with Tommy's role in the story. In The Last of Us Part II, Tommy is the first one who sneaks out of Jackson, with him leaving to find Abby the day after Joel's death. Ellie and Dina are then sent after him, with Jesse showing up later to help.
In HBO's The Last of Us, Ellie and Dina are the first ones to leave Jackson. Up until now, it has been assumed that Tommy is still in Jackson, with many fans wondering how the show will address his part of the story. Episode 5 sees Jesse reveal that he and Tommy actually traveled to Seattle together, with them attempting to reach the city and bring Ellie and Dina back.
3 Dina Is Injured By Seraphites While Going To Lakehill
She Is Shot In The Leg
Ellie, Dina, and Jesse's encounter with the Seraphites changes the Lakehill story in several big ways as well. Firstly, Dina being around at all is a big change. After fighting the Stalkers, the WLF comes after the trio, causing them into the park. In the game, Ellie simply travels through the park, sneakily killing the Seraphites along the way. In the show, the trio unknowingly enters the Seraphite territory, causing the Seraphites to attack them. This fight starts with a Seraphite shooting Dina in the leg with an arrow, injuring her.
2 Ellie Leaves Jesse & Dina During The Seraphite Chase
She Tells Them To Go Back To The Theater
Due to the Seraphite's arrow, Dina can't run anymore. Thus, Ellie comes up with a plan. Ellie tells Jesse and Dina to run back to the theater, with Ellie planning on distracting the Seraphites and then meeting them back there. Ellie then begins wildly firing her gun, with the Seraphites running after her. Ellie eventually escapes, making her way into Lakehill. Jesse and Dina aren't seen again, although it can be assumed that they made it back to the theater. Since Dina never leaves the theater during the Lakehill story, none of this happens in The Last of Us Part II.
1 Ellie & Nora's Fight Scene Plays Out Differently
Thanks To The New Spores
The final change has to do with how Ellie and Nora's fight scene plays out. They both start similarly, with Ellie sneaking up on Nora and holding her at gunpoint. Nora then escapes, causing Ellie to pursue her as the WLF soldiers pursue Ellie. This is where the changes begin. In the game, Nora realizes that she is cornered, having run into a giant hole in the ground. With the WLF soldiers still in pursuit, Ellie takes Nora hostage. Before the soldiers can shoot, Ellie jumps into the hole, taking her and Nora into the spore-filled basement level.

Isaac's merciless torture scene in The Last of Us season 2, episode 4, "Day One," reveals just how vicious the WLF can be against its enemies.
In the show, Nora willingly chooses to enter an elevator shaft, causing her to fall to the basement floors. Ellie doesn't know about spores yet, meaning that her decision to chase Nora down there wasn't a death sentence for Nora. While chasing Nora, Ellie also has to fight off several Infected in the game. In the show, the Infected that are seen have become stuck to the wall due to the Cordyceps. Once Ellie finally reaches Nora again, the dialogue is mostly the same in both versions of The Last of Us.
