Although I do not DS9 in detail all that well after soooooo many years, I am assuming that even if Worf's actions during that mission are part of his permenant record that Starfleet probably put most of the blame of the mission failure on whoever sent a husband and wife out on a mission together. In today's military that would have been a huge no.
