Bellatrix first appears in the flesh in Harry Potter and the Order of the Phoenix. Voldemort's best lieutenant ambushes Harry and his friends after the Dark Lord lures the of Dumbledore's Army to the Department of Mysteries using a false vision of Sirius Black.
Bellatrix taunts Harry, and attacks his friends alongside other Death Eaters, including Lucius Malfoy. She almost succeeds in taking the prophecy from Harry before the mysterious orb is destroyed.
Going To Azkaban Rather Than Denouncing Voldemort
Harry first encounters Bellatrix in a memory of Dumbledore's contained in the Pensieve in Goblet of Fire. Harry watches as the Death Eater proclaims her undying loyalty to Voldemort. She is promptly sentenced to life imprisonment in the wizarding prison, Azkaban, for her crimes.
Several Death Eaters, such Lucius Malfoy and Igor Karkaroff, renounced Voldemort and claimed to be under the influence of the Imperius Curse to avoid an Azkaban sentence. Bellatrix is imprisoned, never swaying in her loyalty to the Dark Lord.

Torturing The Longbottoms For Information About Voldemort's Location
One of Bellatrix Lestrange's most heinous and unforgivable crimes is the torture of Neville's parents, Frank and Alice Longbottom. The Death Eater, along with her husband Rodolphus and brother-in-law Rabastan, used the Cruciatus Curse to torture the Aurors for information regarding Voldemort's whereabouts after his first defeat at the hands of Harry Potter.
Frank and Alice never divulged the vital information, but the Aurors were tortured into insanity by the unforgiving followers of Voldemort. This act appalled the wizarding world and led to Bellatrix's confinement in Azkaban.
Killing Sirius Black
Blood is not thicker than water as far as Bellatrix is concerned. The Death Eater turns on her sister Andromeda for marrying a Muggle-born, and disowns her cousin Sirius as a "blood traitor." Bellatrix eventually kills Sirius during the Battle at the Department of Mysteries.
