During this episode's cliffhanger ending, the Tenth Doctor is shot by a Dalek and begins to regenerate in his TARDIS.

The entire cliffhanger was a huge misdirect, but it certainly tricked plenty of fans into thinking that Doctor Who was actually going to pull off a surprise regeneration.

However, this was quickly revealed to be fake when the following episode opened with the Doctor redirecting his regeneration energy into a severed hand that would later spawn the Meta-Crisis Doctor. The Surprise Would Be Hard To Keep Under Wraps

Although it would certainly be difficult, there's no real reason to believe that Doctor Who couldn't pull off the longed-for surprise regeneration. The main obstacle is that filming for this show often takes place way in advance, with future seasons in production before the ones have even aired. This would make it very hard to keep the secret under wraps, as set photos would ultimately emerge and the actor's identity would surely leak.

However, if the new actor just made a brief appearance in the finale and Doctor Who waited a while before commencing the filming process, it could work. The show basically pulled this off already with Peter Capaldi’s appearance in "The Day of the Doctor"; although he'd already been announced as the replacement prior to Matt Smith's Doctor Who regeneration, nobody knew that he would cameo in the 50th Anniversary special. Similarly, nobody knew that Tennant would appear in "The Power of the Doctor." Thus, Doctor Who could definitely do this again on a larger scale.
